Do you need UK student visa health insurance, or is the surcharge enough?

UK student visa health insurance has two parts: the immigration health surcharge of £776 a year, paid to the Home Office with your visa application, which buys NHS access, and an optional private student policy, which our September 2026 quotes price from £275.13 for a nine-month academic year.

Those two things get confused constantly, and the confusion is expensive in both directions. Students pay for private cover they think is compulsory, or they skip it entirely and discover that the NHS does not pay to fly them home, does not replace a stolen laptop, and does not refund a term of tuition they never got to use. The surcharge is not optional and cannot be swapped for a private policy: GOV.UK states plainly that "You still need to pay even if you have private medical insurance."

What decided our ranking is a clause almost nobody reads. Every student plan in our catalogue is a secondary payer. World Student's policy wording says the insurer reimburses medical expenses "after reimbursement by social security or any other insurance" (terms and conditions, page 10), and Go Campus says "we will reimburse you in addition to the payments made by those organisations" (page 9). Read against the NHS, that is not a weakness, it is the whole point: once the surcharge is paid, a private policy stops competing with the NHS and starts filling the holes the NHS leaves.

So the honest answer for most people on a Student Route visa is that private cover is a genuine top-up, not a duplicate, and it should be bought for what the NHS does not do rather than for a headline medical ceiling you will probably never touch.

If your course collapses, the money you lose is not a hospital bill, it is your tuition. Cap Student is the plan built for that: it refunds the study and accommodation costs you have already paid and cannot use, up to £13,072, and it carries £87,149 of rental liability, which matters the moment you sign for a room in halls or a shared flat. Add £392 of mental health cover and £435.75 a year of routine dental, two things the NHS either rations or charges you for, and it is money well spent for a 273-day year.

If you want the lowest price, World Student is the cheapest for the same year, and it is one of only two plans that accepts students living outside the EU and the EEA. It has no studies interruption cover at all, so you are buying medical and assistance, not tuition protection.

One warning about the top of the market. WorldSecure Basic is the dearest of the five and the thinnest: no studies interruption, no rental liability, and baggage capped at £1,002 against £1,743 elsewhere. Price is not the ranking here.

Two things drive the price and one thing famously does not. Length of stay drives it almost linearly, and your country of residence drives which plans you can buy at all. Age barely moves it: we ran the same nine-month quote at 18, 22, 25, 26, 28, 29, 30, 31, 32, 33 and 34 and four of the five plans returned exactly the same price every time. The exception is Safe Studies, which steps from £336.48 to £396.87 the year you turn 30. Two plans also have a hard ceiling: World Student stops accepting travellers above 34, Go Campus above 35.

The fee

What is the health insurance fee for a UK student visa?

The health insurance fee for a UK student visa is the immigration health surcharge, and it is £776 per person per year for students, student dependants and under-18s. You pay it to the Home Office inside the online visa application, on top of the £558 Student visa fee, and it buys NHS treatment on the same terms as a UK resident.

That £776 rate applies to applications made on or after 6 February 2024 at 9am, according to the Home Office caseworker guidance; before that date the student rate was £470. The standard rate for everyone who is not a student, a student dependant, a Youth Mobility applicant or under 18 is £1,035 a year. The £558 Student visa fee is the figure published on GOV.UK and checked on 1 September 2026, under the Home Office fee table that came into force on 8 April 2026.

You do not pay a flat £776 whatever the course length. GOV.UK sets out the arithmetic: more than six months but under a year costs a full year, eighteen months or less costs "the yearly cost plus half the yearly cost", and between eighteen months and two years you pay "the cost of 2 full years". A three-year undergraduate degree therefore carries roughly £2,328 of surcharge, paid up front, before you have bought a single insurance policy.

The surcharge is not something you can insure your way out of

GOV.UK is explicit: “You still need to pay even if you have private medical insurance.” Any agent or site offering you a private policy as an alternative to the immigration health surcharge is selling you something that will not satisfy the visa application. Buy the private policy for what it adds, never as a substitute.

NHS gaps

What does the UK student visa insurance fee not pay for?

The UK student visa insurance fee buys NHS care, not everything. GOV.UK lists prescriptions, dental treatment, eye tests and assisted conception as services the surcharge does not cover, and the NHS never covers repatriation home, your belongings, your legal liability or the tuition you lose if your year is cut short.

  • GP and hospital care : once the surcharge is paid and your visa has started, you are treated on the same terms as a UK resident.
  • Accident and emergency : emergency treatment is there from day one of the visa, with no ceiling and no upfront payment.
  • Maternity care : covered by the NHS on resident terms, which is more than any of the five student policies offer.
  • Pre-existing and chronic conditions : the NHS treats them. This is the single biggest thing the surcharge buys that private student cover will not.
  • Prescriptions : £9.90 per item in England, according to the NHS, checked September 2026.
  • Dental treatment : £27.90 for a check-up, £76.60 for a filling and £332.10 for a crown or braces under the NHS band charges (the NHS), and urgent treatment is £27.90.
  • Eye tests and glasses : listed by GOV.UK among the services the surcharge does not cover.
  • Getting you home : the NHS treats you in Britain, it does not fly you back to your own country after a serious illness. Every one of the five student plans pays repatriation at actual cost.
  • Your laptop and your belongings : nothing in the NHS covers theft. Baggage cover runs to £1,743 on four of the five plans.
  • Damage you cause : personal liability up to £3.92 million and, on two plans, £87,149 of rental liability for the room you rent.
  • The term you lose : if illness ends your year, the NHS does not refund your tuition. Only Cap Student and Safe Studies do, up to £13,072.

Put the two columns side by side and the shape of a sensible decision appears. The surcharge handles the catastrophic medical risk. A private policy at £275.13 to £352.30 for the year handles the boring, likely, uninsured-by-the-NHS risks: the stolen laptop, the emergency flight home, the dental bill, the flat you flooded.

Short courses

Do courses under six months need UK student visa health insurance?

Yes, and this is the case where private UK student visa health insurance stops being optional. GOV.UK states that "You do not need to pay the surcharge if you're applying for a: visa for 6 months or less from outside the UK", and no surcharge means no NHS entitlement on resident terms.

If you come for a single term, a summer school or a short exchange of six months or less and you apply from abroad, you save the £776 but you arrive with nothing. In our quotes a 96-day pre-sessional course is covered from £124.62 with World Student, or £127.98 with WorldSecure Basic, which is a fraction of what a few days in a British hospital would cost an uninsured overseas visitor.

The Short-term study visa is the confusing middle case. It covers an English language course of six to eleven months, costs £228, and GOV.UK is clear that "You'll also have to pay the healthcare surcharge as part of your online application. It usually costs £776." So an eleven-month language student does get NHS access, while a five-month exchange student applying from abroad does not. Check which of the two you are before deciding how much cover to buy.

EU students

Can EU students claim the UK student visa insurance fee back?

Yes. A full-time student from the EU, Switzerland, Norway, Iceland or Liechtenstein who holds a valid European Health Insurance Card and does not work in the UK can claim the whole immigration health surcharge back from the NHS Business Services Authority, normally within three months.

The conditions are strict and GOV.UK spells them out. You need "a European Health Insurance Card (EHIC) issued in an EU country, Switzerland, Norway, Iceland or Liechtenstein", and "You will not be eligible for a refund if you work or intend to work in the UK while you study." A part-time job in a campus bar is enough to lose the refund.

  1. Pay the surcharge with your visa application. There is no way to skip it at the application stage.
  2. Arrive, enrol, and keep your EHIC valid for the whole of your stay.
  3. Apply to the NHS Business Services Authority for the refund, online for payments made on or after 1 January 2024.
  4. If the claim succeeds, the money goes back to the account that paid the surcharge, normally within three months.
The refund changes what you are covered for

GOV.UK is blunt about the trade-off: after a refund “You will only be able to use your EHIC to access ‘medically necessary treatment’ during your stay.” You move from resident-level NHS entitlement to the narrower EHIC standard, and an EHIC has never covered repatriation, belongings or liability. If you take the £776 back, a private student policy stops being a top-up and becomes the main thing standing between you and a bill.

Fine print

What does UK student visa health insurance hide in the small print?

The five student plans we compared share four traps that no summary sheet shows: they pay only after a state scheme has paid, they exclude pre-existing conditions, they stop at 365 days whatever your degree length, and one of them advertises unlimited medical cover that its own contract caps.

  • They pay second, not first : World Student reimburses "after reimbursement by social security or any other insurance" (wording, page 10) and Go Campus "will reimburse you in addition to the payments made by those organisations" (page 9). With the NHS paying first, expect a top-up, not a cheque for the full bill.
  • Pre-existing conditions are out : World Student excludes "pre-existing illnesses diagnosed and/or treated, resulting in hospitalisation within the six months prior to the request for assistance" (page 15). WorldSecure Basic excludes "congenital disability, hereditary diseases, and chronic illnesses" (page 16). Cap Student excludes "pre-existing illnesses diagnosed and/or treated unless there is an unpredictable complication or clear worsening" (page 36), even though its feature sheet says the opposite.
  • An eight-day waiting period on WorldSecure Basic : "When the Applicant subscribes after his departure, a waiting period of 8 days is applied to all benefits from the date of subscription" (page 2). Buy before you fly and it does not apply.
  • An excess on Go Campus at home : its wording applies an excess of about £26.14 per condition, including VAT, to any care you get back in your own country during the policy (page 9).
  • Everything stops at 365 days : we asked our comparator for a 394-day stay and it returned no offers at all, with every plan rejected for "trip duration too long". Go Campus's wording confirms it: cover runs "up to a maximum of twelve months, renewable once, subject to Assur Travel's prior consent". Renewal is a manual step, not an automatic one.
  • Studies interruption is narrower than it sounds : Cap Student pays only after hospitalisation of more than 30 consecutive days, an insurer-organised medical repatriation, or an insurer-paid early return after a family death, and it requires "prior authorisation from us before returning to your home country" (page 35).

One discrepancy is worth flagging outright, because it is the kind of thing that decides a claim. Cap Student is sold as covering unlimited medical expenses from the first pound, and its summary line in our own catalogue says the same. Its binding contract does not: article 2.4.2.3 on page 10 caps hospitalisation of more than 24 hours at about £871,490 for the whole membership period, and everything else, meaning day cases, GP visits, dental and optical, at about £13,072. On a UK stay where the NHS is doing the hospital work that cap is unlikely to bite, but the marketing line is wrong and the contract is what pays.

One plan we could not check at all. Safe Studies has neither its terms and conditions nor its insurance product information document attached in our system, so nothing on this page is quoted from its wording. We rank it on its published guarantees and price only, and we would rather tell you that than pretend we read a document we do not hold.

The number to check is not the biggest one

On a UK Student Route stay with the surcharge paid, the NHS is your first payer, so the six-figure medical ceiling in the first row is the least likely line to be used. The rows that decide a real claim are studies interruption, rental liability and baggage, and those are exactly the rows where the five plans genuinely differ.

Bank card

Can a credit card replace UK student visa health insurance?

No. Credit card travel cover is written around trips of a few days or weeks, not around a year of living in another country, so it runs out long before a UK Student Route stay does, and it never touches the surcharge.

Two structural gaps matter more than any ceiling. Card policies almost always cap the number of consecutive days abroad they will cover per trip, which a nine-month academic year passes in its first term, and card cancellation cover, where it exists, is limited to a short list of named reasons rather than the broad protection students imagine. We should also be straight about the limits of our own data here: HelloSafe's card database holds no UK-issued card records, so we quote no card ceiling on this page rather than guess at one. When should you buy UK student visa health insurance?

Pay the immigration health surcharge inside the visa application, because there is no other way to pay it, and buy any private UK student visa health insurance before you fly, so no waiting period applies and the policy dates cover your whole stay.

  1. Pay the £776 surcharge with your visa application. It is part of the online form, not a separate purchase.
  2. Work out your real dates, arrival to departure, not term dates. Every quote on this page is priced on the full stay.
  3. Buy the private policy before you leave. WorldSecure Basic applies an eight-day waiting period to anyone who subscribes after departure.
  4. Check the 365-day ceiling against your course. A three-year degree needs a renewal, and on Go Campus that renewal needs the insurer's prior consent.
  5. Keep the certificate. The UK Student visa application does not ask for proof of private insurance, but universities and landlords sometimes do.

  • Not for the UK Student Route: unlike a Schengen visa application, it does not ask for proof of private cover.
