$2,500 medical limit is a real gap for serious injuries

Broke my arm skiing in Japan. ER visit, cast, two follow-up appointments. Total cost: $9,200. Got back $2,450 from Chase after the $50 deductible. Had to cover the other $6,750 out of pocket. $2,500 sounds like coverage but it really isn't for anything involving a hospital outside the US.

Evacuation denied because I didn't pre-authorize

I was hospitalized in Thailand and the doctor recommended air transport to Singapore for specialized care. I arranged it myself and figured I'd file after. Claim denied entirely: pre-authorization from the benefit administrator is required BEFORE transport is arranged, not after. Nowhere does the card marketing make this obvious. Lost $34,000.
