Travel insurance for Spain is not a legal requirement for British visitors, but Spanish border officers may ask to see proof of it, and your GHIC only opens the door of a state hospital: a private clinic asks for payment or proof of cover before admitting you, and nobody flies you home for free. Spain is where Britain goes on holiday: 19.1 million UK residents visited in 2025, more than from any other country, and 32% of UK holidaymakers took a trip there in the year to October 2025 according to ABTA. The bill side is just as British: UK insurers paid £472 million across more than 500,000 travel claims in 2024, medical expenses were the largest category at 34% of cases, and the average overseas medical payout was £1,528.

Three risks shape a Spain policy this autumn: the storms that have been flooding the Mediterranean coast since mid-September, the indefinite strikes running at several Spanish airports, and theft in the big cities. None of them is excluded by the six plans on sale for UK residents, but a cancelled flight is the claim only one of them refunds, and only three pay anything for the delay.
The weather story of October 2026 is rain, not heat. After the hottest and longest summer on record, autumn arrived with a red “danger to life” alert on the eastern coast: on 1 and 2 October 2026, storms dropped more than 250 litres per square metre on Tarragona, cut rail services between Tarragona, Tortosa and Valencia, closed schools for 411,000 pupils and triggered an ES-Alert phone warning in the Ebro delta, with orange warnings extending to Andalusia, Murcia and the Balearics and unsettled weather expected until at least the following Wednesday. For a traveller, a flood is a transport problem first: World Travel pays about £250 for a delayed departure, WorldSecure Platinum and Safe Start about £120, the other three nothing, and no plan refunds a holiday you cancel because of a weather warning.
Airports are the second front. Four strikes were running in Spain in September 2026 with no end date: Groundforce handling staff at Barcelona El Prat since 4 August, Saerco air traffic controllers at nine towers including Lanzarote, Fuerteventura, Seville and Jerez since 17 April, the automated baggage system at Madrid Barajas on recurring two-hour stoppages, and ground cleaning at Málaga since 7 August. On the security side, the FCDO warns that thieves use distraction techniques and often work in teams, with airports during luggage collection and car hire a favourite spot, that demonstrations can start with little warning, and that a number of British nationals have been seriously injured or killed falling from balconies, incidents an insurer may refuse to pay when alcohol or drugs are involved. Theft is a baggage claim, and the baggage clauses differ far more between the six plans than the medical ones do, which is why they get their own section below.

Why this shortlist wins for a Spain holiday
If you fall ill or get hurt in Spain, every plan on this list has the hospital billed directly, so you walk out of a private clinic without advancing a bill that can reach £2,000 a night in intensive care, and every plan flies you back to the UK at actual cost when the assistance doctors decide you should be treated at home. My first pick for a traveller under 66 is WorldSecure Platinum: about £250,000 of medical cover, the baggage cover that actually fits a phone and a camera, about £1,700, search and rescue up to about £4,200 if a hike in the Picos or a swim off Lanzarote goes wrong, and the one winter sports option Sierra Nevada needs. Its limits are an age cap at 65 and a modest excess per claim. Go Explore + is my pick if the medical number is what keeps you awake: about £410,000 of cover with no medical excess at all, accepted to age 80, but with a small baggage ceiling and a phone paid only if it was on you. World Travel is the plan for 66 to 84, the one plan here that still quotes at 81, with about £1,200 of cancellation cover and a delay benefit built in, at the price of a smaller baggage ceiling. Three things move the price. Age first: WorldSecure Basic is allowed only up to 39, WorldSecure Platinum to 65, Safe Start to 69, Go Explore and Go Explore + to 80 and World Travel to 84, so the cheapest quote climbs from £16.07 at 34 to £31.90 at 65 and £52.59 at 72. Duration second: a second week adds £12.66 on Basic and £15.95 on Platinum, while Safe Start, Go Explore and Go Explore + charge exactly the same for 7 and 14 days, which makes Go Explore cheaper than WorldSecure Platinum for a fortnight, £47.72 against £47.85. Party size third: the WorldSecure plans and World Travel price a child like an adult, the Globe Trotter plans behind Go Explore price children lower, which is why a family of four pays £111.99 on Go Explore and £127.61 on Platinum.

Do you need a visa or proof of insurance to enter Spain?
No visa, but possibly proof of insurance: a British passport gets 90 days in any 180-day period in the Schengen area without a visa, and the FCDO lists proof of your travel insurance among the documents Spanish border control may ask for, alongside a return or onward ticket, proof of accommodation and proof of funds. There is no law obliging you to hold a policy and no fine for travelling without one.
That last point needs saying because a story claiming Spain fines British travellers €6,900 (about £5,700) for having no insurance went round in 2025; Which? checked it with UK and Spanish officials and found no such rule exists. What is real is the paperwork: your passport must have been issued less than ten years before you arrive and be valid for at least three months after the day you plan to leave the Schengen area, and the funds rule for 2026 is €122 (about £100) per person per day, with a minimum of €1,099 (about £910) for stays of nine days or more.
Two European systems change the queue, not the rules. The EU Entry/Exit System has registered British visitors' passports, fingerprints and photos at every external Schengen border since 10 April 2026, and the digital record is valid for three years. The ETIAS travel authorisation, a €20 online permit valid three years, has been pushed back to 2027. Neither system asks for insurance, and Spain sells no visa or travel document to a British passport, so this page carries no visa offer.
Health formalities are light: the FCDO health advice for Spain flags ticks, biting insects and altitude sickness in the mountains rather than vaccines, and reminds you that some UK prescription medicines are regulated differently in Spain, so carry them in their packaging with the prescription. Dial 112 for an ambulance. Customs matter more than vaccines: cash of €10,000 or more must be declared when arriving from outside the EU, and the Canary Islands apply the non-EU duty-free limits (one litre of spirits, 200 cigarettes) even on flights from the mainland.
Does a GHIC replace a travel insurance policy in Spain?
No. A GHIC gets you medically necessary state healthcare in Spain on the same basis as a Spanish resident, which the FCDO says is where you are most likely to receive appropriate hospital care, but it is not accepted in a private facility, it does not pay for medical repatriation to the UK, and it does not cover changes to travel and accommodation bookings or treatment ruled non-urgent.
The NHS is explicit: the UK GHIC is free, lasts up to five years, and “is not a replacement for travel insurance”; it names being flown back to the UK and ski or mountain rescue as the two things it never covers. On the Costas and in the islands that gap is wider than it sounds, because the clinic nearest a resort is usually private, and a private clinic will bill you or your insurer, never your GHIC.
- Carry both: The GHIC for a state hospital, the policy for everything else: private clinics, repatriation, baggage, delay.
- Check the card's expiry date: An EHIC issued before Brexit stays valid until its own expiry date; a GHIC lasts up to five years and is free to renew.
- Ask for the state hospital: If you are conscious and have a choice, a state hospital with your GHIC avoids a private bill and an excess claim altogether.
- Keep every receipt: Any co-payment a resident would pay, you pay too; your insurer reimburses it on receipts.
- Private clinics and resort doctors: The GHIC is not accepted; the plan's direct billing is what keeps the bill off your card.
- Repatriation to the UK: Never covered by the GHIC, covered at actual cost on all six plans here.
- Mountain and sea rescue: Never covered by the GHIC; about £2,500 on WorldSecure Basic, £4,100 to £4,200 on four plans and £5,000 on Go Explore +.
- Rebooking a flight or extending a hotel after an illness: Not a GHIC matter at all.
What does a Spanish hospital charge a British visitor who has no cover?
Without a GHIC or a policy, a British visitor pays from the first minute: public hospital emergency triage is billed around €150 to €250 (about £125 to £210) to non-EU patients, a ward night €500 to €800 (about £415 to £665) and an intensive-care night €1,500 to €2,500 or more (about £1,250 to £2,080), according to UrgentDoc's June 2026 price survey. Private emergency departments, including those of the Quirónsalud group in Madrid and Barcelona, start around €200 to €400 (about £165 to £330) for the consultation alone, and the GHIC does not apply there.

Set against those bills, the medical ceiling of the six plans is not the problem: the lowest, about £210,000 on Go Explore, is more than a hundred intensive-care nights, and Safe Start and Go Explore + go to about £410,000. What matters in Spain is who pays first. All six plans include direct billing, meaning the assistance company guarantees the hospital's invoice while you are treated, and the Association of British Insurers' average overseas medical claim of £1,528 in 2024 is roughly three nights on a ward.
The excess is where the plans part company, and it is worth knowing before you reach the counter: the Globe Partner wording behind WorldSecure Basic and the Globe Traveller wording behind WorldSecure Platinum each state an excess of €100 (about £83) per claim; the Globe Trotter wording charges €30 (about £25) per condition on Go Explore and nothing at all on Go Explore +; the Pass and World Travel wordings refer to their tables of benefits without quoting a medical figure. Repatriation is the other large number, and on all six plans the medical evacuation or return to the UK is covered at actual cost, decided by the assistance doctors with the treating physician. Will the policy pay for a phone pickpocketed in Barcelona or Madrid?
Four of the six plans can, on very different terms, and two never do. WorldSecure Basic and Safe Start both exclude mobile phones from their baggage cover outright. WorldSecure Platinum carries about £1,700 of baggage cover with no phone exclusion, the ceiling a phone and a camera fit inside. World Travel pays within about £830, but caps valuables, laptops included, at 30% of that amount and wants the police complaint within 24 hours. Go Explore and Go Explore + pay for a phone or laptop only if it was in a safe or on you when it was taken, within about £210 and £410 respectively, with the theft reported to the police within 48 hours and the claim sent within five working days.
Pickpocketing without violence is the standard Spanish theft, and the FCDO's warning about teams working distraction techniques at airports and car hire desks is the scenario to insure. The Globe Partner wording behind WorldSecure Basic lists mobile phones, laptops, jewellery and audio and video devices among excluded items; the Pass wording behind Safe Start excludes mobile phones and covers jewellery and audio and video devices only when worn or locked in a safe, with an excess of €50 (about £42) per suitcase. Every wording asks for a theft report filed with a competent authority, and compensation is paid at replacement value less depreciation, on receipts.
- Baggage ceiling that fits your kit: WorldSecure Platinum, about £1,700, is the plan where a phone and a camera fit inside the limit without a valuables cap.
- Police report, the same day: File a denuncia at a Policía Nacional or Guardia Civil station and keep the stamped copy: World Travel wants it within 24 hours, Go Explore and Go Explore + within 48 hours, and no insurer pays without it.
- Phone on you, not in the bag: On Go Explore, Go Explore + and World Travel a phone is paid when it was carried on you; one left in a beach bag or a hotel room drawer is not.
- Proof of ownership: Compensation is paid at replacement value less depreciation on receipts; photograph your receipts before you leave.
- Phones on WorldSecure Basic and Safe Start: Named exclusions, whatever the circumstances of the theft; Basic adds laptops and audio and video devices.
- Valuables above 30% on World Travel: Jewellery, watches and laptops together cannot exceed 30% of the £830 baggage amount, about £250.
- Items left unattended: A bag left on a beach or in a car in a public place is excluded on World Travel and on the Pass wording.
- Items you cannot prove you owned: No receipt, no photo, no replacement value to pay.
What changes for Tenerife, the Canary Islands, Mallorca and Ibiza?
Nothing in the price and nothing in the plan list: a Spain policy covers the Canary Islands and the Balearics as part of Spain, so a Tenerife, Gran Canaria, Mallorca or Ibiza holiday quotes exactly like Madrid, £16.07 for a week on WorldSecure Basic for a 34-year-old. What changes is the risk mix: sun, sea and rented scooters, and the fact that the nearest clinic is usually private, where the GHIC does not count.
Distance is the first difference. On an island, a case the local hospital cannot handle means an air transfer to the mainland, which is where the repatriation and evacuation benefit earns its place: actual cost on all six plans. At sea, the search and rescue benefit matters: about £5,000 on Go Explore +, £4,100 to £4,200 on WorldSecure Platinum, Safe Start, Go Explore and World Travel, and £2,500 on WorldSecure Basic, figures the GHIC never pays.
Rented scooters and mopeds are the second. The Globe Traveller wording behind WorldSecure Platinum excludes accidents on a motorcycle over 125 cc, as driver or passenger, from its accidental death and disability cover; the 50 and 125 cc rentals of Ibiza and Mallorca sit under that line. The Pass wording behind Safe Start goes further and excludes any motor sport, as rider or passenger, and any accident while driving over the local alcohol limit. No wording makes a helmet or a licence a written condition, but the local police do, so ride within your licence.
The third is the sun, and the winter. Heat illness is covered as any illness, but a day in the Tenerife or Andalusia sun in July is the most common reason a visitor ends up in a private emergency department, at €200 to €400 (about £165 to £330) for the consultation alone. Skiers in Sierra Nevada, open from 28 November 2026, or in Baqueira-Beret, need the winter sports option, sold on WorldSecure Platinum only: with the option ticked, Platinum is the one plan the comparison returns.
Which clauses in a Spain policy catch British travellers out?
Five clauses do most of the damage on Spain claims: an excess per claim, a pre-existing condition exclusion, age caps from 39 to 84, cancellation cover that exists on one plan only, and motor and alcohol exclusions. All five come from the policy wordings we read, not from the sales sheets.
- Excess per claim: €100 (about £83) per claim on WorldSecure Basic and WorldSecure Platinum, €30 per condition on Go Explore, none on Go Explore +; Safe Start charges €50 per suitcase on baggage.
- Pre-existing conditions: Every wording excludes them; the Globe Trotter and Pass wordings name any condition hospitalised in the six months before departure. Declared conditions need a specialist UK policy, compared in our pre-existing conditions guide below.
- Age caps: WorldSecure Basic to 39, WorldSecure Platinum to 65, Safe Start to 69, Go Explore and Go Explore + to 80, World Travel to 84. From 85 no plan quotes for UK residents on this comparison.
- Cancellation on one plan only: World Travel includes about £1,200 of cancellation cover for listed reasons; the other five neither include nor sell it to a UK resident.
- Motorcycles and motor sports: Over 125 cc excluded from WorldSecure Platinum's accident cover; any motor sport excluded on Safe Start.
- Alcohol: The FCDO warns that insurers may not pay for incidents involving alcohol or drugs, balcony falls first among them, and the Pass wording behind Safe Start excludes drunkenness and driving over the limit in so many words.

Why the shortlist still works: the excesses are small against Spanish private clinic bills, the age caps are published and the comparison applies them for you (a 40-year-old simply never sees WorldSecure Basic), and four of the six plans can be bought up to the day you fly. Go Explore and Go Explore + must be bought at least the day before departure, so buy the day before all the same and the certificate is in your inbox when you reach the airport.
Which plans refund a prepaid Spain holiday you can no longer take?
One of the six does, within a limit: World Travel includes about £1,200 of cancellation cover, paid when you cancel because of an accident or illness with hospitalisation, a death, the death of a close relative in the 15 days before departure, theft of your identity documents in the 48 hours before departure, a visa refusal or your home becoming uninhabitable. Strikes, transport breakdowns, missing documents and airline decisions are not covered, and nor is a change of mind. WorldSecure Basic, Safe Start, Go Explore and Go Explore + have no cancellation cover, and WorldSecure Platinum does not sell its cancellation option to UK residents on this comparison. What you do have as a British holidaymaker is the package travel framework: a package booked with an ATOL-protected organiser is refunded if the organiser fails, and the organiser must refund you if it cancels or significantly changes the holiday. That protection covers their failure, not your change of plans, your illness or a weather warning, which is exactly the gap a cancellation policy fills.
- Add up what you have prepaid per person: flights, accommodation, car hire. Under about £1,200, World Travel's built-in cover is enough for the listed reasons.
- Check the reason you are most likely to cancel for against World Travel's list above: illness with hospitalisation and bereavement are in, a strike or a storm warning is not.

What else do British travellers ask about insurance for Spain?
Not by law: Spain does not require British visitors to hold a policy and there is no fine for travelling without one, whatever the 2025 stories said. The FCDO does list proof of travel insurance among the documents Spanish border control may ask for, and your GHIC only covers state healthcare, not a private clinic, repatriation or baggage. No. It gives you state healthcare on the same terms as a Spanish resident, which is valuable, but it is not accepted in private clinics, it never pays for medical repatriation to the UK or for mountain and sea rescue, and it does nothing for a stolen phone or a cancelled flight. The NHS says in so many words that it is not a replacement for travel insurance.
A state hospital will, on the same terms as a resident. A private clinic, which is usually the nearest one in a resort, will not accept the GHIC and will ask for payment or an insurer’s guarantee before admission. All six plans on this page include direct billing, so the assistance company guarantees the invoice while you are treated.
On WorldSecure Platinum, yes, within about £1,700 of baggage cover and against a police report. World Travel pays within about £830 with valuables capped at 30%, Go Explore and Go Explore + pay for a phone only if it was on you, and WorldSecure Basic and Safe Start exclude mobile phones from baggage cover entirely.
Yes. The Canary Islands and the Balearics are Spain for all six plans, and the quote is identical to a mainland trip. The clauses to check there are search and rescue at sea, from about £2,500 on WorldSecure Basic to £5,000 on Go Explore +, and the motorcycle exclusions if you rent a scooter: over 125 cc on WorldSecure Platinum, any motor sport on Safe Start.
The Entry/Exit System has registered British visitors at Schengen borders since 10 April 2026; there is nothing to apply for, your passport, fingerprints and photo are taken at the border and the record lasts three years. ETIAS, the €20 online authorisation valid three years, has been postponed to 2027. Neither requires travel insurance.

World Travel includes about £1,200 of cancellation cover for listed reasons: illness with hospitalisation, death, bereavement in the 15 days before departure, theft of identity documents, visa refusal or an uninhabitable home. The other five plans sell no cancellation cover to a UK resident on this comparison. A costlier prepaid package or villa needs a separate cancellation policy, compared on our trip cancellation insurance guide; ATOL protection refunds you only if the organiser fails, not if you cancel.
Yes, as an illness: the emergency medical cover of all six plans applies, and we found no heat or sunstroke exclusion in the wordings we read. What is not covered is cancelling your holiday because a heatwave or storm warning has been issued.
On four plans, yes: WorldSecure Basic, WorldSecure Platinum, Safe Start and World Travel accept a purchase up to departure day. Go Explore and Go Explore + must be bought at least the day before. Buy the day before in every case, so the certificate is in your inbox before you reach the airport.
It depends on the plan: €100 (about £83) per claim on WorldSecure Basic and WorldSecure Platinum, €30 per condition on Go Explore, and none at all on Go Explore +. Safe Start and World Travel refer to their tables of benefits. When the hospital is billed directly, the excess is the only amount you pay.
