To file a claim, you will need an itemized invoice from the veterinarian, a completed claim form, and possibly your pet’s full medical history. Claims can be submitted digitally via the MyEmbrace portal, email, or by mailing documents to Embrace Pet Insurance Claims Dept, P.O. Box 22188, Beachwood, OH 44122-0188. Claims are generally processed within 10 days and reimbursement is paid by check or direct deposit.

Which veterinarians can I use with an Allstate pet insurance policy?

You can visit any licensed veterinarian, specialist, or emergency clinic in the US or even worldwide. There are no network restrictions or required referrals for covered services. Simply pay at time of service and submit your claim for reimbursement.

What is covered and not covered with Allstate pet insurance?

Covered services include accidents, illness, cancer, surgeries, lab work, hospital stays, chronic and genetic conditions, medications, and behavioral therapy if veterinarian-prescribed. Preventive care such as checkups, vaccines, spay/neuter, and dental cleaning is only reimbursed if you have purchased the optional Wellness Rewards add-on. Pre-existing conditions, elective/cosmetic procedures, breeding, and most supplements or personal care items are excluded.
