Travel insurance for Portugal starts at $21.39 for a week, and it earns its money on three bills the state system never pays: the flight home, treatment in a private clinic, and the cost of searching for you at sea or in the mountains.
Portugal reads as the safest holiday in Europe, and mostly it is. What catches travellers out is geography rather than danger. The busiest beaches face open Atlantic rather than a sheltered sea, the resort strips of the Algarve are served heavily by private clinics that bill on the spot, and two of the country's regions are island groups a long flight from the mainland. None of those three facts is a reason not to go. All three are reasons the cheapest policy is not automatically the right one.
We priced this page on the live comparator on 29 August 2026 for a real trip: one traveller aged 34, seven days in Portugal, departing 3 October 2026. From a British residence six plans came back, from $21.39 to $84.25 for the whole week. From an American residence a different set of seven came back, five of them sold by the day from $5.16. Two findings came out of those runs and they shape everything below: the plan list follows where you live rather than where you are going, and not one plan in the British set will sell cancellation cover for a Portugal trip.

You want to walk into a clinic in Albufeira or Funchal with a broken wrist and walk out without paying, and you want somebody else to organise and fund the flight home if it comes to that. Every plan below does the first part: direct billing is included on all six, so the clinic invoices the insurer instead of your card, and repatriation is written as actual costs, which means there is no ceiling on it.
What separates them is what Portugal specifically throws at you.
WorldSecure Platinum is my first pick at $42.46 for the week, and the only one I would buy if there is any chance of a hire scooter, a hard hiking day or a sport the wording does name: it is the single plan in the British set that keeps cover when the extreme sports option is ticked, and the tick costs $2.02 more. It also carries about $5,890 of search costs, written into the contract as the cost of searching at sea or in the mountains.
Safe Start at $51.84 has the biggest medical ceiling of the European set, about $589,000, plus a stay interruption benefit of about $14,140 if you are sent home early. World Travel at $69.99 is the one that still quotes for a traveller of 84 and carries the highest transport delay cap, about $353. Why does travel insurance for Portugal change with your country of residence?
Travel insurance for Portugal is sold by where you live, not by where you are going, so a British and an American traveller on the same Faro flight see different plans at different prices. On 29 August 2026, only two of the plans available for a Portugal trip were offered to both.
We ran the identical week in Portugal twice, changing nothing but the country of residence. From the United Kingdom, six plans priced. From the United States, seven, five of them sold as a daily rate rather than a trip premium. The reason is regulatory rather than commercial: an insurer is authorised to sell into specific markets, so the comparator filters the catalogue by your residence before it prices anything at all.

The sharpest consequence is cancellation. When we re ran the Portugal week with the cancellation option switched on, an American resident kept three plans, from $5.16 a day. A British resident kept none: every plan in the British set returned "cancellation coverage not supported" for this trip. We reproduced that with six different booking dates, from 20 August through to the day of the quote, before writing it down. Set your real country of residence in the comparator before you read any price, on this page or anywhere else. A Portugal quote built for a French resident is not a quote you can buy from Manchester or Boston, and the plan names will not even match.
Does a GHIC or EHIC replace travel insurance for Portugal?
No. A GHIC or EHIC gives a British traveller medically necessary state healthcare in Portugal on the same terms as a Portuguese resident, but the NHS Business Services Authority states flatly that the card "never covers repatriation costs", and it does not pay for a private clinic.
Portugal is in the EU and in the Schengen area, so the card genuinely works there and gets you into the Servico Nacional de Saude, the Portuguese state health service. The trap is that the card and the policy cover different things, and in Portugal the gap between them opens exactly where most holidays happen. Resort areas and the smaller islands lean heavily on private clinics, and a private clinic is outside the card by definition.
- Being flown back to the UK : named by the NHS as a service the card does not cover, and by the NHS Business Services Authority in absolute terms. A stretcher flight with a medical escort is arranged and paid by an insurer, never by a health card.
- Treatment in a private medical facility : also named by the NHS. In the Algarve and on Madeira the clinic you can actually reach is often the private one, and it will bill you.
- Ski or mountain rescue : the NHS names rescue explicitly. Portugal's hardest walking is on Madeira's levadas and the volcanic trails of the Azores, where a rescue is a real invoice from a real operator.
- Statutory patient charges : the NHS warns that "not all state healthcare is free outside of the UK" and that you may pay what a local resident would pay. The card puts you in the queue, it does not make the queue free.
- Changes to travel and accommodation bookings : listed by the UK government on its Portugal advice. If illness forces you to rebook flights or extend a hotel, the card contributes nothing.
So carry both. The card removes the state hospital bill, the policy removes everything the card leaves behind. American travellers have no equivalent card in Portugal at all, which is why the US plans in the table above lead with a $250,000 to $500,000 medical ceiling rather than a small top up. Does travel insurance for Portugal cover surfing and watersports?
Yes, on the contracts we opened. Recreational surfing is not named as an excluded or extreme sport in any of the three policy wordings behind the Portugal shortlist, so a surf lesson in Ericeira or a hire board in the Algarve is covered by travel insurance for Portugal as an ordinary leisure sport.
This matters more in Portugal than almost anywhere else in Europe. Ericeira, on the coast north of Lisbon, was dedicated as a World Surfing Reserve in October 2011, the first in Europe and the second in the world, protecting five kilometres of coastline and breaks including Ribeira d'Ilhas and Coxos. A surf week is a mainstream Portuguese holiday, not a fringe one, and travellers routinely assume it needs a specialist policy.

We read the binding wordings rather than the summary sheets, because the summaries do not resolve this. WorldSecure Platinum excludes a closed list of leisure sports unless the risk sports option is bought: "bobsleigh, skeleton, mountaineering, competitive luge, aerial sports with the exception of parasailing, sea scooters, combat sports, off-piste sliding sports, scuba diving with tanks". WorldSecure Basic excludes "mountaineering, rock-climbing, bobsleigh, skeleton, any air sport, jet ski, martial arts, off-piste winter sports". Smart Travel Protection, the American plan, defines adventure and extreme activities as a closed list running from B.A.S.E. jumping to "scuba diving if the depth exceeds 131 feet (40 meters)". Surfing, bodyboarding and stand-up paddle appear on none of the three lists.
What those same lists do name is the motorised and the deep. Sea scooters are excluded by WorldSecure Platinum, jet skis by WorldSecure Basic, and free diving by Smart Travel Protection. So the honest rule for Portugal is that the wave is fine and the engine is not: paddling out on a board is a leisure sport, towing yourself around a bay on a powered craft is the thing the contract wrote down.
There is no separate form field where you declare a surfboard. Ticking the extreme sports option in the comparator is the declaration, and on a Portugal trip it costs you the whole market: switch it on and a British resident is left with one plan, WorldSecure Platinum at $44.48 for the week, while an American resident is left with Smart Travel Protection from $5.16 a day. Every other plan returns "extreme sports coverage not supported". Tick it if your week includes something the wording actually names. Do not tick it reflexively for a surf lesson.
The sea itself deserves more respect than the policy does. The UK government warns that on Portuguese beaches "waves can be unpredictable and some beaches have a strong undertow", and that on supervised beaches a red flag means no entry and a yellow flag means wading only. Does travel insurance for Portugal pay to search for you at sea or in the mountains?
On the European plans, yes, up to a stated ceiling. WorldSecure Platinum's contract pays "the cost of searching at sea or in the mountains following an event that puts your life at risk", up to about $5,890, and travel insurance for Portugal sold to a British resident carries that benefit on all six plans.
This is the benefit almost nobody compares, and in Portugal it is the one that matches the map. The country's two signature outdoor activities, Atlantic surf and island hiking, are precisely the two that generate a search rather than an ambulance. The condition attached is narrow and worth knowing before you need it: the contract reimburses "only fees charged by a duly authorised company for these activities", so an official rescue operation is in scope and an informal search by the people you were staying with is not.

Read that table as two mirror images rather than a ranking. Every European plan in the Portugal set carries search and rescue and none records a medical evacuation at sea. Smart Travel Protection is the only plan in the set whose feature record carries evacuation at sea, and it is the one with no search and rescue line at all. If you are American and your Portugal week is built around the ocean, that asymmetry is the single most useful thing on this page: the plan that will lift you off the water is not the plan that will pay to look for you.
None of this is covered by a health card. The NHS lists "ski or mountain rescue" among the services a GHIC or EHIC does not cover, which puts the entire benefit on the insurance side of the line for a British walker on Madeira.
What changes about travel insurance for Portugal on Madeira and the Azores?
Two things change: the walking is harder than on the mainland, and any serious case has to leave the island by air. Travel insurance for Portugal covers both, but the American plan we read pays the transfer only when "adequate Medically Necessary treatment is not available in Your immediate area".
The UK government's advice is blunt about the terrain: "hiking in Madeira and the Azores can be challenging. Choose paths that suit your fitness and experience." Madeira's levada paths run through the Laurissilva, a forest that has been a UNESCO World Heritage site since 1999 and that covers around 15,000 hectares, roughly 20% of the island. The evacuation clause is where the island geography bites. Smart Travel Protection pays emergency medical evacuation up to $500,000 to "the nearest suitable Hospital or medical facility where Medically Necessary treatment is available", but only once "the local attending Physician and Our designated Travel Assistance Services Provider determine that Your condition is acute, severe or life threatening" and that treatment is unavailable in your immediate area. On an island that test is easier to meet than on the mainland, which is the good news, and it is also why calling the assistance line first is not optional: the transfer has to be arranged by them.
One threshold is worth checking against the map before you buy. Smart Travel Protection classes "Mountain Climbing over 9,000 feet (2,700 meters)" as an extreme activity. Mount Pico, on the Azorean island of the same name, is the highest point in Portugal at 2,351 metres, or 7,713 feet. It sits below that line, so the summit walk is not an extreme activity under that wording. Nothing in Portugal reaches the contract's ceiling, which is a rare case of a threshold that resolves an entire destination in your favour.
On the European plans the island leg is simpler still, because repatriation is written as actual costs rather than a stated ceiling, so a transfer from Funchal or Ponta Delgada is not separately capped. On the American plans it is capped: $500,000 on Smart Travel Protection and $400,000 on Standard Plan.
What conditions does travel insurance for Portugal hide in the small print?
Three conditions actually disqualify Portugal travellers, and none of them appears in a plan's headline: the age cap, the cancellation gap and the extreme sports tick. Two of the six plans in the British set refuse a 68 year old outright, and none of the six will sell cancellation cover for a Portugal trip at all.
- Check the age cap before the price : WorldSecure Basic stops at 39 and WorldSecure Platinum at 65, so both vanish for a traveller aged 68 and only four plans still quote, from $51.84. World Travel goes to 84, Go Explore and Go Explore + to 80, Safe Start to 69.
- Decide about the extreme sports tick deliberately : ordinary surfing does not need it on the wordings we read, it costs $2.02 more on WorldSecure Platinum for a week, and it is the only way to cover a sport the contract does name.
- Confirm direct billing rather than reimbursement : all six European plans and both American plans in our tables include it, which is what stops an Algarve private clinic asking for your card at the desk.
- Read the medical excess, not just the ceiling : Safe Start applies about $35 per condition and World Travel about $59, while WorldSecure Platinum and WorldSecure Basic apply none at all.
- Weigh the search benefit if you are walking or surfing : Go Explore + carries the highest at about $7,070 and WorldSecure Basic the lowest at about $3,540, a gap far wider than the price difference between them.
- Assuming you can add cancellation cover later : from a British residence no plan in our Portugal set returns cancellation cover at all, reproduced across six booking dates. An American resident keeps three plans, from $5.16 a day.
- Reading a baggage ceiling as a per item limit : Go Explore caps the entire baggage benefit at about $295 with an excess of about $29, which one camera exhausts. WorldSecure Platinum reaches about $2,360, Safe Start the same, and World Travel about $1,180.
- Expecting a wildfire to trigger a refund : WorldSecure Platinum's terms state that assistance cannot intervene where the request is consecutive to "pollution, natural disasters", and WorldSecure Basic excludes "accidents caused by hurricane, earthquake, volcanic eruption or other disasters". Book flexible accommodation for July and August instead of relying on the policy.
- Renting a bigger bike than the contract allows : WorldSecure Platinum's accidental death and disability cover excludes "accidents caused by the use of a motorcycle with a cylinder in excess of 125 cm3, either as driver or passenger". Two gaps matter more than the ceilings. First, the money flows the wrong way: the Ritz-Carlton benefit states in its own conditions that the "cardholder must advance all costs", which is precisely the situation an Algarve private clinic creates and precisely what the direct billing on a real policy exists to avoid. Second, cancellation on a card runs on a short closed list of covered reasons. Across the same 128 cards, 37 cover trip cancellation, 19 carry cover we rate as misleading and 64 do not cover it at all.
One honest limitation: the HelloSafe card database holds no UK issued cards, so a British reader cannot check their own card against these numbers here. What entry rules apply alongside travel insurance for Portugal?
Portugal asks British and American tourists for no visa and no proof of cover at the border, so travel insurance for Portugal is a financial decision rather than a legal one. What is changing is the EU's border technology, not the insurance requirement.
British and American passport holders travel to Portugal visa free for short stays as Schengen third country nationals, subject to the 90 days in any 180 rule counted across the whole Schengen area rather than per country. Madeira and the Azores are part of Portugal and inside that same area, so an island leg does not restart the clock.
