Is digital nomad health insurance worth it for Americans abroad?

Digital nomad health insurance is worth it for most Americans working abroad: Medicare and most marketplace plans stop at the US border, while a dedicated nomad plan starts at $2.53 a day and covers hospital bills up to about $587,000. Real HelloSafe quotes, August 2026.

The difference from vacation cover is structural. A two-week trip needs cancellation and emergency protection. Six months in Chiang Mai or Lisbon needs a medical ceiling that can absorb a real hospitalization, access to everyday doctor visits, a way to renew without flying home, and cover for the laptop that earns your living. The CDC Yellow Book prices a medical evacuation at $25,000 within North America and $250,000 or more from remote locations, which is exactly the tail risk this insurance exists to remove.

To write this guide, we priced every plan sold to US residents on real 6-month and 12-month stays, read the full policy wording behind our top pick (terms plus the insurance product information document), and pulled the exact ceilings, waiting periods and sub-limits into the sections below.

Our methodology

Every year, our experts audit each plan against its binding policy wording: coverage, limits and exclusions, cross-checked with real customer feedback and live market prices. The analysis is fully independent, with zero paid placements and no insurer able to pay to influence a rating. We compare travel insurance across every country and currency, using the plans, prices and cover actually offered in your country, not a single guide translated.

Health vs travel

How is digital nomad health insurance different from travel insurance?

Digital nomad health insurance is long-stay medical cover: it pays hospital bills, doctor visits and medicine while you live and work abroad for months at a time. Standard travel insurance protects a short trip, with cancellation, baggage and emergency-only medical cover.

Most products sold to nomads sit somewhere between the two. On the US market, Smart Travel Protection is trip protection first: it refunds up to $40,000 of prepaid travel if you cancel, and adds $250,000 of emergency medical. Epic Plan and Standard Plan are built for long adventurous trips, with up to $500,000 of emergency medical and 250 plus covered activities. Go Explore is the closest to true health cover: it reimburses consultations, lab tests, pharmacy and teleconsultation at 100% of actual costs from the first dollar billed, not only emergencies.

Our expert's take for digital nomads

You walk into a clinic in Bangkok or Mexico City, show your certificate, and the consultation, tests and pharmacy are reimbursed at 100% of actual costs: that is what sets Go Explore apart for a nomad, on top of hospital cover of about $293,000, direct billing and repatriation at actual costs, for $456.26 over 6 months. Go Explore + doubles the medical ceiling to about $587,000 for $648.84. If you prefer a dollar-denominated contract, Atlas International carries $500,000 of medical and $1,000,000 of evacuation from a starting rate of $5.90. Epic Plan is the pick when your work funds an adventurous lifestyle, with 250 plus activities covered. One honest caveat: Go Explore caps laptop and phone theft at about $117, so insure serious equipment separately.

Two patterns are worth noting. First, a year costs 1.6 times a half-year on the budget plan, not double, so a longer certificate saves money when your plans are firm. Second, the premium tier buys ceiling rather than extras: for the solo 30-year-old, $192.58 more over 6 months doubles the medical cap.

Coverage

What should digital nomad health insurance cover?

Digital nomad health insurance should cover emergency hospital care of at least $250,000, medical evacuation, everyday outpatient care and personal liability, with honest sub-limits on equipment. Here is what our US shortlist actually pays, from the offer sheets and policy wording.

  • Emergency medical of $250,000 or more : the US shortlist runs from $250,000 (Smart Travel Protection) and $400,000 (Standard Plan) to $500,000 (Atlas International, Epic Plan) and about $587,000 (Go Explore +).
  • Medical evacuation : $500,000 on Smart Travel Protection, $1,000,000 on Atlas International; Go Explore repatriation is paid at actual costs.
  • First-dollar outpatient care : on Go Explore, GP and specialist consultations, lab tests, radiology, pharmacy and teleconsultation are reimbursed at 100% of actual costs, with no deductible.
  • Direct billing in hospital : Go Explore advances hospital costs for stays over 24 hours, so you never front the bill; emergency dental adds about $352 per illness and $705 after an accident.
  • Personal liability : about $1,760,000 worldwide on Go Explore, reduced to about $587,000 for claims in the USA and Canada, with a deductible of about $176.

Equipment is the weak spot of the whole category. Go Explore's dedicated electronics guarantee tops out around $117, and Epic Plan's $3,000 baggage ceiling is the highest in the set. If a $2,000 laptop earns your living, budget a separate equipment policy rather than counting on any of these plans.

Fine print

What does digital nomad health insurance not cover?

Digital nomad health insurance does not cover routine prevention, pregnancy, unstabilized medical conditions or professional sport, and equipment sub-limits are tight. Every exclusion below comes from the policy wording behind our top pick, read in full in August 2026.

  • Pregnancy and scheduled hospital stays : both are excluded on Go Explore, along with spa and cosmetic treatment.
  • Unstabilized pre-existing conditions : anything still under treatment with a risk of sudden deterioration is excluded; our pre-existing condition travel insurance guide covers the options that remain.
  • Preventive and long-term care : checkups, chronic-condition management and prosthetics (dental, optical, hearing) are out of scope on every plan in the set.
  • Laptop or phone theft above about $117 : paid only if the device was locked in a safe or carried on you, with a deductible of about $29 per claim.
  • Professional sport : amateur practice is fine and Epic Plan covers 250 plus activities, but paid competition is excluded everywhere; see our extreme sports travel insurance guide for high-risk hobbies.
Buy before you fly, and check the age caps

Taken out once you are already abroad, the cover behind Go Explore applies a 5-day waiting period before benefits start; bought before departure, it starts on your departure date with no waiting. The same contract stops enrolling travelers over 65 for stays in the USA and Canada, and over 80 everywhere else.

Long stays

How do renewals and home visits work with digital nomad health insurance?

Digital nomad health insurance can follow you for up to 36 months abroad: Go Explore renews on written request before expiry, keeps paying up to about $23,500 of medical costs during temporary visits home, and ends on your definitive return to the US.

  1. Ask for renewal in writing before your current certificate expires.
  2. The broker checks you still meet the eligibility conditions and confirms the price.
  3. Cover continues into the new period; the 36th month spent abroad ends insurability for good.

Trips home are the detail most nomads miss. During a temporary return to the US, Go Explore keeps covering medical and hospital costs up to about $23,500, with an excess of about $35 per condition. A definitive return ends the cover, and policies longer than one month carry a 14-day cooling-off period after signature if you change your mind.

If you left without insurance, two plans in our comparison, Epic Plan and Standard Plan, can be bought or extended while you are already traveling. For nomads who chain shorter trips instead of one long stay, annual travel insurance is usually the cheaper structure, and our when to buy travel insurance guide shows which guarantees expire if you wait.

US gap

Does US health insurance cover digital nomads abroad?

In most cases, no: US health insurance does not follow digital nomads abroad. Medicare usually does not cover health care outside the United States, and marketplace plans are built on domestic networks that end at the border.

Medicare lists only narrow exceptions, such as transit through Canada between Alaska and another state. The State Department tells travelers to confirm their coverage before departure, and the CDC Yellow Book is blunt: Medicare pays neither for care abroad nor for evacuation. Keeping your US plan still makes sense for visits home and for your eventual return; the nomad policy covers the months in between.

One legal exception is worth knowing: Schengen visa applicants must show at least €30,000 of medical cover, an amount fixed in euros by EU rules. Atlas International is flagged as Schengen-compliant in our data. For the wider market beyond nomad-friendly plans, start from our travel insurance hub.

FAQ

Digital nomad health insurance: your questions answered

  • On real August 2026 quotes for a 30-year-old US resident, Go Explore works out at $76.04 a month over a 6-month stay ($456.26 in total) and $59.24 a month over a full year ($710.88 in total). Go Explore + doubles the medical ceiling for $108.14 a month over 6 months.

  • Yes, with conditions. Epic Plan and Standard Plan can be bought or extended while you are already traveling. Go Explore can also start abroad, but a 5-day waiting period applies before benefits begin; bought before departure, it starts immediately.

  • Go Explore does: GP and specialist consultations, lab tests, radiology and pharmacy are reimbursed at 100% of actual costs up to the plan ceiling. Preventive checkups and chronic-condition management remain excluded on every plan we compare.

  • Usually yes, if you can afford both. The nomad policy covers you abroad and up to about $23,500 during temporary visits home, but it ends on your definitive return, so dropping US cover entirely means re-enrolling from scratch when you move back.

  • The contract behind Go Explore stops enrolling travelers over 65 for stays in the USA and Canada, and over 80 everywhere else. Age also moves the price: the same 6-month stay costs $456.26 at age 30 and $566.12 at age 45.

  • Stabilized conditions can be insurable, but anything still under treatment with a risk of sudden deterioration is excluded, and so is pregnancy. Complete the application accurately: the contract lets the insurer suspend cover when declarations are wrong.

  • Travel insurance with strong medical cover: Epic Plan carries $500,000 and Standard Plan $400,000 of emergency medical, plus 250 or more covered activities. Neither pays for everyday non-emergency care, so they suit trip-style nomads more than settled remote workers.

  • At least €30,000 of medical cover valid across the whole Schengen area, an amount fixed in euros by EU rules. Atlas International is flagged Schengen-compliant in our comparison data, and a visa certificate is the document consulates ask for.

  • Insurability on Go Explore ends with your 36th month abroad, renewals included. Past that point you are in expat territory: a permanent international health plan, which is a different product from the temporary long-stay cover compared on this page.
